Legal instrument for distribution of property and care of dependents in the event of your death.

Power of Attorney

Statement detailing your desires in relation to medical treatment if you are no longer able to express informed consent.

Individual or Family Trust

Legal instrument authorizing someone else to make health care decisions for you upon satisfaction of specific factors.
